NECESSARY DOCUMENTS

US Club Roster
Here are the steps to finding this roster through GotSport:
1. Log in to your GotSport account
2. Click Team Management
3. Select your team
4. Click Rosters
5. Choose your roster (select your US Club roster then Search)
6. Click on Documents & IDs and then Roster PDF (save this file to your desktop for next step).
7. Use this roster to upload to GotSport

MAKING CHANGES TO THIS ROSTER:

PLAYERS NOT ATTENDING: If you have players that are not attending, please cross out the players on this roster before uploading.

ADDING GUEST PLAYERS: If you need to add a guest player, please contact your club registrar and they should be able to add the guest player to this US Club roster, and then you can follow the steps above to download that roster again.

Player Cards
A team must have player cards for all players on their roster.  All players mush have player cards from the same sanctioning organization as the team's roster.   For example, a player carded under US Youth Soccer cannot play with a US Club sanctioned roster and vice versa.  No roster may be comprised of players with different passes from different sanction organizations.

Here are the steps to find your player cards:
1. Log in to your GotSport account
2. Click Team Management
3. Select your team
4. Click Rosters
5. Choose your roster (select your US Club roster then search)
6. Click on Documents and then ID Cards and it will download a pdf with player cards for all your players (save this file to your desktop for next step)
7. Upload this pdf to GotSport as your player cards

US Club Guest Player Form (only if you add guest players)
If you add a guest player be sure to upload their guest player form as well as the player card.

Medical Release Form (DO NOT NEED TO UPLOAD)
We do not need to see Medical Release Forms at registration, however it is mandatory that each player have a completed Medical Release form with them at each game.  This is your standard medical release form provided by your club.
