Academy Roster (must be signed by club registrar)
If you already have your Academy roster with your club registrar signature on it, please upload this roster to GotSport.  Approved roster will be emailed back to you.

If you need a blank Academy roster, please CLICK HERE.  Fill out this form and get your club registrar to sign it.  Once you have the signed Academy roster, please upload this roster to GotSport.  Approved roster will be emailed back to you.

NTSSA Academy Forms
This replaces player cards for U7-U10 Academy teams registered through North Texas State Soccer Association.

If you already have your NTSSA Academy Forms for each player, please upload them to GotSport.  If you need help gathering these forms, please contact your Home Association where the player is registered OR login to their player account to download a copy.

*Important to note that individual parents/players can access their NTSSA Academy Forms by logging in to their player accounts and downloading a copy.

Please have them send you their NTX Academy Forms and once you have all Forms for all players, please upload them to GotSport.

Guest Player Form (only if you add guest players)
If you add a guest player be sure to upload their guest player form as well as their NTSSA Academy Form

Medical Release Form (DO NOT NEED TO UPLOAD)
We do not need to see Medical Release Forms at registration, however it is mandatory that each player have a completed Medical Release form with them at each game.
